The prescribed form for a writ of election referred to in subsection 39(1) of the Act is set out in Form 1 of the Schedule.

The prescribed form for a proclamation referred to in subsection 44(1) of the Act is set out in Form 2 of the Schedule.

The prescribed form for a ballot referred to in subsection 110(2) of the Act is set out in Form 3 of the Schedule.

The prescribed form for a special ballot referred to in subsection 111(1) of the Act is set out in Form 4 of the Schedule.
